چکیده

Telmisartan is a potent, long-lasting, nonpeptide antagonist of the angiotensin II type-1 (AT1) receptor that is indicated for the treatment of essential hypertension. It selectively inhibits stimulation of the AT1 receptor by angiotensin II without affecting other receptor systems involved in cardiovascular regulation. Very high lipophilicity a unique feature of telmisartan coupled with a high volume of distribution indicate that the compound offers the clinically important advantage of good tissue penetration. It also activates peroxisome proliferator–activated receptor c (PPAR-c) and increase adiponectin protein content in adipocytes. By this means they may improve insulin sensitivity. It belongs to a class II drug in BCS classification i.e. low solubility and high permeability. One of the major problems with this drug is its low solubility in biological fluids which results into poor bioavailability after oral administration to improve the aqueous solubility and dissolution rate of the telmisartan solid dispersions of drug using different methods were prepared and investigated. Enhancement of solubility of Telmisartan was observed with solid dispersion of drug using carriers such as Poly vinyl pyrrolidonek30, Poly ethylene glycol-4000 and βeta –Cyclodextrin, poly ethylene glycol 6000, alkalizers, Gelucire 43/01, Poloxamer 407, PVP K30 and HPMC E4 , PEG 6000 and NaHCO3. The observed results showed the solid dispersion of drug greater than the pure drug.

The vasodilatory action of telmisartan on isolated mesenteric artery rings from rats

Objective(s): Angiotensin Ⅱ type 1 receptor blockers (ARBs) represent one of the widely used antihypertensive agents. In addition to anti-hypertension effect, some ARBs also show other molecular effects such as activating peroxisome proliferator-activated receptor-γ and so on. Here we studied the effects of telmisartan on the rat isolated mesenteric artery rings pre-contracted by phenylephrine ...

Renoprotection by Telmisartan versus Benazepril in Streptozotocin Induced Diabetic Nephropathy

Diabetic nephropathy (DN) is one of the major causes of end stage renal disease. Angiotensin converting enzyme (ACE) inhibitor and angiotensin receptor blocker (ARB) are preferred for delaying progression of DN. This study compared the preventive renal effects of telmisartan (10 mg/kg, p.o.), an ARB that completely blocks angiotensin action, and benazepril (5 mg/kg, p.o.), an ACE inhibitor, whi...

Comparative review of the blood pressure-lowering and cardiovascular benefits of telmisartan and perindopril

Hypertension is a major cardiovascular (CV) risk factor, and blood pressure (BP)-lowering treatment substantially reduces the risk. This review compares the available clinical evidence from the BP-lowering and CV-outcome studies of telmisartan and perindopril, which are among the most intensively studied members of their respective classes. The PubMed database was searched for telmisartan and p...

Does the inhibition of renin-angiotensin system decrease inter-dialytic weight gain in anuric hemodialysis patients?

OBJECTIVE Knowledge about the inhibition of centrally located angiotensin-I (AT-I) receptors by highly lipophilic AT-I receptor blockers and its' effect are limited with experimental studies. Thus, we aimed to investigate the effect of Telmisartan on Inter-dialytic weight gain (IDWG) % and echocardiographic measurements in anuric hemodialysis (HD) patients. PATIENTS AND METHODS A total of for...

Systematic review of the effect of telmisartan on insulin sensitivity in hypertensive patients with insulin resistance or diabetes.

WHAT IS KNOWN AND OBJECTIVE Telmisartan is an angiotensin receptor blocker (ARB) originally developed for the treatment of hypertension. It can also partially activate peroxisome proliferator-activated receptor (PPAR)-γ, which may improve insulin sensitivity. This effect may prove useful in hypertensive patients with insulin resistance or diabetes mellitus. Such activity is more marked than tha...

Critical appraisal and pooled analysis of telmisartan alone or in combination with hydrochlorothiazide for achieving blood pressure goals

Rigid control of blood pressure (BP) is essential to prevent cardiovascular disease. However, only about 40% of hypertensive patients undergoing pharmacological intervention with a single agent achieve their BP goals in contemporary clinical practice. Combined therapy using currently available agents is effective in maximizing treatment outcome, although it raises medical costs and decreases th...
